This "Request for Prepayment of Premiums" for a life insurance policy is filled out in an unknown hand but signed at the close with a magnificent "George H. Ruth" autograph. The single page measures 5.25x8.5" in size and exhibits original minor folds and a paper clip impression at the upper left, neither of which cause any significant distraction. A marvelous example of a rare and early full name Ruth autograph variation.
